Generator pocket air after EV-ICD implantation linked to transient defibrillation threshold elevation.
Case Report
Air in the generator pocket after extravascular ICD implantation may be associated with transient defibrillation threshold elevation.
EV-ICD adds ATP to non-transvenous defibrillation; leaves open comparative efficacy and safety versus S-ICD.
In addition to the subcutaneous implantable cardioverter-defibrillator (S-ICD), the extravascular implantable cardioverter-defibrillator (EV-ICD), which uses a substernal lead, has recently been introduced into clinical practice as a non–transvenous ICD system. The EV-ICD enables defibrillation without intravascular or intracardiac leads and is distinguished by its ability to provide additional therapeutic options, including antitachycardia pacing (ATP).

Morita et al. (2026) conducted a case report in Extravascular ICD implantation. Extravascular ICD implantation was evaluated. Transient defibrillation threshold elevation was associated with air in the generator pocket after extravascular ICD implantation.
