Temporally resolved WSS and nwTKE were strongly correlated (ρ = 0.64), while cycle-average nwTKE was weakly correlated to OSI (ρ = −0.273), and hemodynamic parameters were only weakly correlated to vessel diameter and bifurcation angle.

Why the study?

Different expressions of hemodynamic shear stress are involved in carotid atherosclerosis pathogenesis, highlighting the need to characterize and compare their differential impact and geometric relationships in the atherosclerotic carotid bifurcation.

Population

245 subjects with atherosclerotic plaques of at least 2.7 mm in SCAPIS

Comparison

Relationships between hemodynamic shear stresses (WSS, nwTKE, OSI) and carotid geometry

Design

Cross-sectional imaging study using 4D Flow MRI and contrast-enhanced MRA

Overview

nwTKE may complement WSS and OSI for disturbed flow assessment in carotid plaques; leaves open its prognostic value pending prospective studies.

Structured PICO

P
Population
179 adults (analyzed out of 245 enrolled) between 50 and 65 years old with at least one asymptomatic carotid plaque larger than 2.7 mm, recruited as part of the Swedish CArdioPulmonary bioImage Study (SCAPIS). Mean age 59.0, 73% male.
O
Outcome
Characterization and comparison of hemodynamic wall shear stresses (WSS), near-wall turbulent kinetic energy (nwTKE), and oscillatory shear index (OSI) in the carotid arteries, and their relationships with vessel diameter and bifurcation angle.surrogate

In vivo 4D Flow MRI demonstrates a strong correlation between turbulent and mean components of wall shear stress in carotid atherosclerosis, while showing that near-wall turbulent kinetic energy provides complementary information to oscillatory shear index not strongly predicted by vessel geometry.

Limitations

  • MR-based WSS underestimates true WSS due to poor spatial resolution
  • MRI-based measurements are subject to noise and segmentation errors
  • No comparison against CFD for error propagation analysis
  • No cohort of subjects without atherosclerosis available for comparison
  • No follow-up examinations to determine clinical implications
