Implication

No VTE risk increase with antipsychotics in elderly; leaves open residual confounding and need for prospective confirmation.

Structured PICO

Does antipsychotic drug use increase the risk of venous thromboembolism in elderly patients?

P
Population
Elderly patients with at least 1 antipsychotic drug prescription during 1998 to 2008 (cohort n=111,818). Nested case-control included 1,032 VTE cases (367 hospital admissions for DVT, 342 for PE, 323 outpatient DVT treatments) and 4 age- and sex-matched controls per case.
I
Intervention
Antipsychotic drug use (evaluated as current, recent, or past use, duration of use, and type of antipsychotic drug)
C
Comparator
Nonusers of antipsychotic drugs
O
Outcome
Venous thromboembolism (VTE), defined as hospital admission for deep venous thrombosis (DVT) or pulmonary embolism (PE), or outpatient start of therapeutic dose low-molecular weight heparin therapyhard clinical

In elderly patients, the use of antipsychotic drugs does not appear to be associated with an increased risk of venous thromboembolism.
