Wide QRS complexes immediately following PSVT termination occur in approximately 14% of episodes and are associated with markers of ischemia and drug termination, likely representing ventricular arrhythmias.
PSVT停止直後に出現するwide QRS complexについて検討した.対象は1分間以上持続し, かつ頻拍停止時の記録が確認しえた96回 (男性42回, 女性54回, 平均年齢56歳) の自然発作PSVTで, wide QRS complexの出現頻度と心電図所見を検討し, ついで頻拍周期, 頻拍持続時間, 頻拍前後のST偏位, 及び頻拍停止方法について, 出現群 (W群) と非出現群 (N群) の間で比較検討した.wide QRS complexは13例 (14%) に認められ, 心室不整脈である可能性が強く, 9例が連発例であった.N群, W群の比較では, W群は頻拍周期が短く, 頻拍持続時間は長く, 頻拍前後のST変化が大きかった.また薬剤による停止例に多かった.PSVT停止後のwide QRS complexの出現には, 頻拍中の循環不全及び停止薬が関連している可能性が考えられた.
