Overview

Vitamin D3 does not improve endothelial function or arterial stiffness in healthy postmenopausal women; challenges prior observational links to cardiovascular benefit.

Structured PICO

Does Vitamin D3 supplementation improve endothelial function and arterial stiffness in healthy post-menopausal women?

P
Population
114 healthy, community-dwelling, ambulatory post-menopausal women from Madison, Wisconsin with serum 25-hydroxyvitamin D concentrations between 10 and 60 ng/mL. Mean age 63.9 years, 100% female.
I
Intervention
Vitamin D3 2500 IU oral daily (administered in a cookie) for 4 months
C
Comparator
Identical tasting placebo cookie oral daily for 4 months
O
Outcome
Changes in brachial artery flow-mediated vasodilation (FMD), carotid-femoral pulse wave velocity (PWV), and aortic augmentation index (AIx) after 4 monthssurrogate

Vitamin D3 supplementation at 2500 IU daily for 4 months did not improve endothelial function, arterial stiffness, or inflammation in healthy post-menopausal women, suggesting it may not reduce cardiovascular risk.

Limitations

  • Endpoints are surrogate markers for CVD risk, not hard clinical events
  • Participants were generally healthy post-menopausal women with typical 25(OH)D levels, limiting generalizability to men or those with higher CVD risk
  • Optimal dose and duration of vitamin D supplementation remain debated, and higher doses or longer duration might be needed
