Overview

Hypercholesterolemia-linked loss of insulin venodilation is hypothesis-generating; prospective studies needed before any clinical implications.

Structured PICO

Is insulin-mediated venodilation impaired in patients with hypercholesterolemia?

P
Population
40 subjects total: 22 healthy, normotensive hypercholesterolemic subjects (HC) with normal glucose tolerance, and 18 age-matched normal control subjects (NC).
I
Intervention
Infusion of insulin (8, 16, 24, and 32 microU/min) combined with norepinephrine (100 ng/min) into dorsal hand veins. A subset of 12 HC also received lovastatin 20 to 40 mg/d for 6 weeks.
C
Comparator
Age-matched normal control subjects (for the primary comparison of venoreactivity).
O
Outcome
Changes in venous diameter measured by ultrasonography.surrogate

Hypercholesterolemia impairs insulin-mediated, endothelium-dependent venodilation, which is not reversed by short-term cholesterol reduction with lovastatin.
