Implication

Transient RAAS activation in decompensated HF warrants caution against indefinite intensification; leaves open optimal timing of inhibition in severe dysfunction.

Structured PICO

P
Population
23 patients with severe congestive heart failure (NYHA class III-IV) divided into three groups: recent onset severe pump failure with pulmonary edema (n=5), recent cardiac decompensation superimposed on chronic CHF (n=9), and chronic stable CHF (n=9). An additional 9 patients with acute, apparently uncomplicated myocardial infarction were also studied.
O
Outcome
Plasma renin activity (PRA) and plasma aldosterone levels across different clinical states of heart failuresurrogate

The renin-angiotensin-aldosterone system is activated during acute decompensation of heart failure, likely driven by decreased systemic blood pressure and cardiac output, but normalizes upon clinical stabilization.
