A rare, but important, complication of mitral valvotomy for mitral stenosis is the development of tricuspid incompetence The syndrome may be defined as permanent tricuspid incompetence, coming on in the immediate post-operative period, after a successful mitral valvotomy, without any obvious cause for sudden right heart failure, in a patient who had never had tricuspid incompetence before operation, or in whom it had been transient or of slight degree. In this paper an account is given of the syndrome, with a discussion of its mechanism, treatment, and prognosis.
