To investigate skeletal muscle myofibrillar protein metabolism in heart failure patients, focusing on myosin heavy chain (MHC) synthesis, content, and isoform shifts, and their ties to inflammation and functional capacity.
Evaluated 10 patients with chronic heart failure (age 63 ± 3 years) and 11 healthy controls (age 70 ± 3 years).
Assessed skeletal muscle MHC and actin protein content, fractional MHC protein synthesis rates, MHC isoform distribution, circulating C-reactive protein, and physical performance (strength and exercise capacity).
MHC protein content decreased with worsening disease severity in heart failure patients (r = -0.748, P < 0.02) and strongly correlated with reduced exercise capacity and muscle strength, whereas actin content was unaffected.
Heart failure patients demonstrated a lower relative abundance of MHC I (P < 0.05), and systemic inflammation via C-reactive protein negatively correlated with MHC protein synthesis (r = -0.442, P = 0.05) and the MHC-to-mixed muscle synthesis ratio (r = -0.493, P < 0.03).
