To compare the risk of ipsilateral stroke between patients presenting with a first-ever retinal transient ischemic attack versus a hemispheric transient ischemic attack in the setting of medically treated high-grade carotid stenosis.
Analyzed 129 medically treated patients with 70% to 99% carotid stenosis who entered the randomized North American Symptomatic Carotid Endarterectomy Trial with a first-ever transient ischemic attack.
Compared 59 patients presenting with retinal transient ischemic attacks (RTIAs) against 70 patients presenting with hemispheric transient ischemic attacks (HTIAs).
The 2-year Kaplan-Meier risk of ipsilateral stroke was 16.6% ± 5.6% for RTIAs compared with 43.5% ± 6.7% for HTIAs (P = .002).
Hemispheric transient ischemic attacks conferred a markedly higher risk of ipsilateral stroke compared to retinal attacks across 75% to 95% stenosis, with an overall relative risk of 3.23 (95% CI, 1.47 to 7.12).
Patients with RTIAs experienced a longer mean delay from symptom onset to medical treatment initiation compared to patients with HTIAs (48.5 vs 15.2 days).
