An inverted-L-shaped partial sternotomy with peripheral cardiopulmonary bypass and systemic hyperkalemia allowed for safe and successful aortic valve replacement without injuring a functional right internal thoracic artery graft.
Case Report (n=1)
An inverted-L-shaped partial sternotomy with peripheral cardiopulmonary bypass and systemic hyperkalemia is a safe approach for re-do aortic valve replacement in patients with a functional RITA graft close to the sternum.
May support partial sternotomy for re-do AVR with patent RITA grafts; hypothesis-generating and requires confirmation in larger series.

Furukawa et al. (2012) conducted a case report in Aortic valve stenosis after previous CABG with functional RITA graft (n=1). Inverted-L-shaped partial sternotomy with peripheral cardiopulmonary bypass and systemic hyperkalemia was evaluated on Successful aortic valve replacement without RITA graft injury. An inverted-L-shaped partial sternotomy with peripheral cardiopulmonary bypass and systemic hyperkalemia allowed for safe and successful aortic valve replacement without injuring a functional right internal thoracic artery graft.
