Implication

Epinephrine addition may induce maternal vasodilation and hypotension; leaves open randomized confirmation of clinical impact in cesarean anesthesia.

Structured PICO

Does the addition of epinephrine to bupivacaine for epidural anesthesia alter maternal hemodynamics in term gravidas undergoing cesarean section?

P
Population
32 term gravidas undergoing elective cesarean section
I
Intervention
Bupivacaine (0.5%)-epinephrine (5 μg/ml) mixture administered via lumbar epidural in 5-ml increments to a T4 level following i.v. hydration with 2000 ml Ringer's lactate (n=16)
C
Comparator
Plain bupivacaine administered via lumbar epidural in 5-ml increments to a T4 level following i.v. hydration with 2000 ml Ringer's lactate (n=16)
O
Outcome
Hemodynamic changes (systolic, diastolic, mean pressures, systemic vascular resistance, cardiac output, stroke volume, heart rate) measured at T10 and T4 sensory levelssurrogate

Epidurally administered epinephrine with bupivacaine produces mainly beta-2 mediated vasodilatation, significantly decreasing blood pressure and systemic vascular resistance without altering cardiac output or heart rate.
