症例は72歳男性.咳嗽,左前胸部痛を主訴に当院を受診した.来院時の胸部ctにて左胸腔内に巨大腫瘤が認められ,腫瘤による縦隔偏位および左肺気管支の圧排による無気肺が認められた.エコーガイド下に針生検を施行し,組織診断の結果,脱分化型脂肪肉腫が疑われた.手術は胸腔鏡下に開始し,腫瘤を摘出する際に左第4肋間開胸を施行した.術中所見では腫瘍は前縦隔より発生しており,左肺および胸壁などの周囲組織への浸潤は認められなかった.病理学的精査の結果は脱分化型脂肪肉腫の診断であった.今回われわれは前縦隔に発生した巨大腫瘤を呈した脱分化型脂肪肉腫の切除例を経験した.腫瘍切除に際して,腫瘍が巨大であるため通常開胸では視野確保が困難であり,胸腔鏡下操作が有用であった一例と考えられた.
