A scoring system combining medical history and conventional echocardiographic parameters predicted low global longitudinal strain (<16%) with an area under the curve of 0.86, 80.3% sensitivity, and 76.5% specificity.
Why the study?
Detection of LV abnormalities is essential to delay progression in preclinical HF, and while GLS is a sensitive marker for early subtle abnormalities, not all echocardiographic instruments can measure it.
Does a scoring system predict left ventricular longitudinal myocardial dysfunction in patients with preclinical heart failure?

Does a scoring system predict left ventricular longitudinal myocardial dysfunction in patients with preclinical heart failure?
Effect estimate: AUC 0.86 (95% CI 0.83-0.88)
A simple scoring system based on sex, LVEF, and E/e' can evaluate LV longitudinal myocardial dysfunction in preclinical heart failure, which may be useful when global longitudinal strain measurement is unavailable.
May support GLS estimation via simple score in preclinical HF when unavailable; leaves open prospective validation before adoption.
BACKGROUND: Detection of left ventricular (LV) abnormalities is essential for patients with preclinical heart failure (HF) to delay progression to clinical HF. Global longitudinal strain (GLS) is a sensitive marker for the early occurrence of subtle abnormalities in LV function, but not all echocardiographic instruments can measure GLS. METHODS AND RESULTS: [female]=1 point, LV ejection fraction <59%=2 points, E/e' >14=1 point). CONCLUSIONS: Our scoring system provides an easy-to-use evaluation of LV longitudinal myocardial dysfunction, and may prove useful for risk stratification of patients with preclinical HF.

Odajima et al. (2024) conducted a cohort in Preclinical Heart Failure (Stage A and B) (n=853). Scoring system for predicting low global longitudinal strain was evaluated on Prediction of low global longitudinal strain (GLS <16%) (AUC 0.86, 95% CI 0.83-0.88).
