Does a primary care based disease management program improve health-related quality of life in patients with congestive heart failure?
A primary care based disease management program for CHF patients is associated with significant improvements in health-related quality of life as measured by the SF-36 at 1 year.
Sidorov et al. (2003) conducted an observational in Congestive heart failure (n=268). Primary care based disease management program vs. Baseline (no control group) was evaluated on SF-36 scores on eight subscales and two composite scales. A primary care-based disease management program for congestive heart failure significantly improved 3 of 8 SF-36 subscales and the Mental Health Composite Score at 1 year.
