Concurrent systemic hypertension alters the hyperdynamic cardiovascular state induced by portal hypertension in cirrhotic patients.
Why the study?
Portal hypertension causes systemic hemodynamic changes including decreased SVRI, increased CI, and narrowed Ca-vO2, but the relationship with liver cirrhosis severity and local circulation is not fully characterized.
In patients with portal hypertension and liver cirrhosis, the presence of systemic hypertension attenuates the typical systemic hyperdynamic circulation state.
Concurrent hypertension may mask hyperdynamic circulation in cirrhosis; hypothesis-generating for hemodynamic interactions.

Chikamori et al. (2012) conducted a review in Portal hypertension and liver cirrhosis (n=60). Systemic hypertension in cirrhosis vs. Normotensive cirrhosis was evaluated. Portal hypertension induces a hyperdynamic systemic state characterized by increased cardiac index and decreased systemic vascular resistance, which is significantly altered in cirrhotic patients with concurrent systemic hypertension.
