Ticagrelor improves flow-mediated dilation by ~33% vs high-dose clopidogrel in diabetic CAD patients post-PCI.
Why the study?
Patients with diabetes mellitus treated with PCI have high residual platelet reactivity and endothelial dysfunction; the effect of enhanced platelet inhibition on endothelial function is unknown.

Does ticagrelor improve endothelial function compared to high-dose clopidogrel in diabetic patients treated with percutaneous coronary intervention?
Absolute Event Rate: 16% vs 12%
p-value: p=0.009
In diabetic patients with stable coronary artery disease post-PCI, maintenance therapy with ticagrelor significantly improves endothelial function compared to high-dose clopidogrel.
Supports preferring ticagrelor over high-dose clopidogrel for endothelial function post-PCI in diabetes; extends P2Y12 head-to-head data with FMD endpoint.

Mangiacapra et al. (2016) conducted an RCT in Type 2 diabetes mellitus and stable coronary artery disease post-PCI (n=42). Ticagrelor vs. Clopidogrel 150 mg once daily was evaluated on Flow-mediated dilation (FMD) at 14 days (T1) (p=0.009). Ticagrelor significantly improved flow-mediated dilation compared to high-dose clopidogrel (16.0% vs 12.0%, P=0.009) in diabetic patients with stable coronary artery disease post-PCI.
