Key result
Cardiac allograft ectopic lymphoid structures are linked to higher local IgG and ~19% donor-specific antibody detection.
Why the study?
Cardiac allograft vasculopathy involves ectopic lymphoid structures producing antibodies, but the antigenic targets of these locally produced antibodies were not characterized.
Population
56 heart transplant recipients with coronary arteries and epicardial tissue collected at autopsy
Design
Observational study with immunofluorescence, ELISA, Luminex, and qPCR analyses

May indicate ELS role in local DSA-mediated rejection; leaves open whether targeting these structures improves graft outcomes.
Observational (n=56)

Ectopic lymphoid structures in cardiac allografts contain active plasma cells that can locally produce donor-specific HLA antibodies, potentially contributing to graft rejection and cardiac allograft vasculopathy.
Huibers et al. (2016) conducted an observational in Cardiac allograft vasculopathy (n=56). Ectopic lymphoid structures (ELS) vs. Smaller ELS or no ELS was evaluated on IgG and IgM levels and donor-specific HLA antibodies (p=< 0.05). Ectopic lymphoid structures in cardiac allografts were associated with higher local IgG levels (p<0.05), with donor-specific HLA type II antibodies detected locally in 19% of patients with ELS.
