Does pindolol compared to metoprolol affect cardiac function and blood pressure differently in hypertensives with incipient heart failure?
Pindolol and metoprolol provide similar blood pressure reduction and left ventricular function effects in hypertensive patients with incipient heart failure, though pindolol causes less reduction in heart rate and cardiac index due to its intrinsic sympathomimetic activity.
Both agents support BP control without LV function differences in this population; confirms beta-blocker equivalence with ISA modulating heart rate effects.
1 This study was designed to compare in a randomized cross-over trial pindolol, a beta-adrenoceptor blocking agent with intrinsic sympathomimetic activity and metoprolol, a cardioselective beta-adrenoceptor blocker lacking sympathomimetic activity in hypertensives with incipient heart failure. Cardiac function was investigated by M-Mode and 2D-echo-cardiography. 2 Both pindolol and metoprolol in equipotent doses lowered blood pressure to the same extent. 3 There was no difference in overall left ventricular function in ventricular geometry or in wall dynamics after either drug. 4 In comparison to metoprolol, pindolol produced less reduction in resting heart rate, cardiac index and ejection velocity reflecting the intrinsic sympathomimetic activity of the drug.

Hahn et al. (1982) conducted an RCT in hypertensives with incipient heart failure. pindolol vs. metoprolol was evaluated on blood pressure and left ventricular function. Pindolol and metoprolol lowered blood pressure to the same extent and showed no difference in overall left ventricular function, though pindolol produced less reduction in resting heart rate.
