Key result
RV bifocal pacing is linked to a ~6% LVEF increase following unsuccessful biventricular pacing.
Why the study?
Does bifocal right ventricular pacing improve clinical and echocardiographic outcomes in patients with heart failure and unsuccessful biventricular pacing?
Observational (n=39)
No
Does bifocal right ventricular pacing improve clinical and echocardiographic outcomes in patients with heart failure and unsuccessful biventricular pacing?
Absolute Event Rate: 29.4% vs 23.3%
p-value: p=0.013
Bifocal right ventricular pacing may serve as an acceptable alternative for heart failure patients with RBBB who fail to respond to classical biventricular pacing.
Should not change practice in BiV non-responders; leaves open benefit of bifocal RV pacing pending prospective trials.

Occhetta et al. (2016) conducted an observational in Heart failure with indication for cardiac resynchronization therapy (n=39). Right ventricular bifocal pacing vs. Baseline was evaluated on Left ventricular ejection fraction at 12 months (p=0.013). Right ventricular bifocal pacing significantly improved left ventricular ejection fraction from 23.3% to 29.4% at 12 months in heart failure patients with unsuccessful biventricular pacing.
