Overview

May support early serial cTnT sampling in acute PE; leaves open prospective validation for risk stratification.

Structured PICO

What are the release kinetics of cardiac troponin T in survivors of confirmed severe pulmonary embolism compared to patients with myocardial microinfarction?

P
Population
9 consecutive survivors of angiographically confirmed acute pulmonary embolism (PE) who developed cTnT ≥0.1 μg/L, mean age 65 years, 6 males. PE was moderate in 3 and severe in 6.
I
Intervention
Serial blood sampling for cardiac troponin T (cTnT) measurement (on admission, every 4 h for 24 h, and daily until discharge)
C
Comparator
6 control patients with confirmed acute coronary syndromes and microinfarction (cTnT ≥0.1 μg/L with normal ECG and CK-MB)
O
Outcome
Release kinetics of cardiac troponin T (cTnT), including time to peak, peak concentration, and duration of elevationsurrogate

Cardiac troponin T release in acute pulmonary embolism peaks earlier and resolves faster than in acute coronary syndromes, suggesting a different mechanism of myocardial damage and highlighting the need for early serial sampling for risk stratification.

Limitations

  • Cannot answer whether cTnT originates from the cytosolic pool or a different cell pool, or whether release is attributable to severe reversible or irreversible myocardial ischemia
