Implication

Scar-focused ablation may be considered in LVAD VT; hypothesis-generating and leaves open need for randomized confirmation.

Structured PICO

P
Population
34 patients (30 male, age 58±10 years) with a Heart Mate II continuous flow left ventricular assist device (LVAD) who underwent ventricular arrhythmia catheter ablation at 9 tertiary centers. Underlying cardiomyopathy was ischemic in 21 and nonischemic in 13, with mean LVEF 17%±5% before LVAD implantation.
I
Intervention
Ventricular arrhythmia catheter ablation (39 procedures targeting 110 VTs and 2 VF triggers)
O
Outcome
Ventricular arrhythmia characteristics, ablation targets (cannula vs intrinsic scar), and arrhythmia-free survival

Catheter ablation of VT in patients with continuous flow LVADs is feasible and safe, with intrinsic myocardial scar being the dominant arrhythmogenic substrate rather than the device cannula.
