Key result
Intravenous clonidine (3 µg/kg) was significantly more effective than metoprolol (2 mg/kg) in attenuating the rise in heart rate and blood pressure 1 minute after intubation (p=0.0001).
Why the study?
Anaesthesia induction, laryngoscopy, and tracheal intubation trigger marked haemodynamic and autonomic reflex changes that are concerning in high-risk patients.
Does clonidine improve the attenuation of hemodynamic responses to intubation compared to metoprolol in normotensive patients undergoing elective surgery?
RCT (n=60)

Does clonidine improve the attenuation of hemodynamic responses to intubation compared to metoprolol in normotensive patients undergoing elective surgery?
Intravenous clonidine is more effective than metoprolol in attenuating the hemodynamic stress response (tachycardia and hypertension) to laryngoscopy and endotracheal intubation.
Clonidine or metoprolol may blunt intubation responses in elective normotensives; leaves open comparative efficacy and safety in high-risk patients.
Background: The sequence of induction of anaesthesia, laryngoscopy and tracheal intubation are associated with marked haemodynamic changes and autonomic reflex activity which may be a cause of concern in many high risk patients.Aim: Objective of this study is to compare effectiveness of clonidine & metoprolol in attenuation of intubation response. Methods:In this study,60 ASA I and II status normotensive patients scheduled for elective surgical procedure were selected randomly and divided into two groups of 30 each. All the patients received standard premedication Tab. Alprazolam 0.5 mg at bed time and were fasted for 12 hours. Group I received 3 µgm/kg I.V Clonidine and Group II 2 mg/ kg Metoprolol 90 seconds before intubation. Results: After intubation,Clonidine caused more better control in Heart rate than the pre-op value and was also very effective than Metoprolol in minimizing the rise in BP after intubation which is statistically significant and desirable. Conclusion:Though Clonidine,Metoprolol were effective in obtunding the raise in blood pressure (SBP,DBP,MAP ),Clonidine was much better than Metoprolol in controlling blood pressure.

Parveen et al. (2023) conducted an RCT in Normotensive patients scheduled for elective surgical procedure (n=60). Clonidine vs. Metoprolol 2 mg/kg I.V. was evaluated on Heart rate at 1 minute after intubation (p=0.0001). Intravenous clonidine (3 µg/kg) was significantly more effective than metoprolol (2 mg/kg) in attenuating the rise in heart rate and blood pressure 1 minute after intubation (p=0.0001).
