A patient with hypertrophic cardiomyopathy experienced an inappropriate shock from a subcutaneous implantable cardioverter-defibrillator due to atrial oversensing of fibrillatory waves.
Case Report (n=1)
Patients with hypertrophic cardiomyopathy may be uniquely susceptible to inappropriate S-ICD shocks from atrial oversensing of fibrillatory waves, warranting careful pre-implantation screening.
May warrant enhanced atrial sensing screening before S-ICD in hypertrophic cardiomyopathy; leaves open generalizability beyond this case.
KEY TEACHING POINTS•Inappropriate shocks in subcutaneous implantable cardioverter-defibrillators are known to occur from oversensing of ventricular events (T-wave oversensing and rapid ventricular rates falling into the shock zone) or noise.•We report a patient with hypertrophic cardiomyopathy who had inappropriate shock for atrial fibrillation due to atrial oversensing of fibrillatory waves, which has not been described before.•Patients with hypertrophic cardiomyopathy may be uniquely susceptible to atrial oversensing due to atrial hypertrophy and high R-wave slew rate; careful screening is warranted before subcutaneous implantable cardioverter-defibrillator implantation. Karnik et al. (2017) conducted a case report in Hypertrophic cardiomyopathy (n=1). Subcutaneous implantable cardioverter-defibrillator was evaluated on Inappropriate shock due to atrial oversensing of fibrillatory waves.
