Key result
Administration of the ACE inhibitor enalaprilat and the AT1 receptor antagonist candesartan did not affect the hypoxic pulmonary vasoconstriction response in anaesthetized dogs.

ACEIs/ARBs may not impair HPV in acute canine models; leaves open translation to human critical care hypoxemia.
Does angiotensin-converting enzyme inhibition or type 1 angiotensin II receptor blockade inhibit hypoxic pulmonary vasoconstriction in anaesthetized dogs?
Endogenous angiotensin II does not appear to mediate acute hypoxic pulmonary vasoconstriction in intact dogs, suggesting that ACE inhibitors or ARBs may not worsen gas exchange by inhibiting this mechanism in critically ill patients.
Hubloue et al. (2004) studied Hypoxic pulmonary vasoconstriction (n=16). Enalaprilat and Candesartan vs. Baseline (pre-drug administration) was evaluated on Increase in Ppa-Ppao gradient (mean pulmonary artery pressure minus occluded pulmonary artery pressure) in response to hypoxia. Administration of the ACE inhibitor enalaprilat and the AT1 receptor antagonist candesartan did not affect the hypoxic pulmonary vasoconstriction response in anaesthetized dogs.
