Key result
Mitral valve replacement with the Epic or Mosaic prosthesis yielded satisfactory results, with the Mosaic valve potentially offering better freedom from reintervention (P=0.067).
Why the study?
Comparative studies of clinical outcomes between different biological mitral valve prostheses are scarce.
Does mitral valve replacement with the Epic bioprosthesis compared to the Mosaic bioprosthesis improve freedom from reintervention and overall survival in patients undergoing isolated elective MVR?

Does mitral valve replacement with the Epic bioprosthesis compared to the Mosaic bioprosthesis improve freedom from reintervention and overall survival in patients undergoing isolated elective MVR?
p-value: p=0.067
In patients undergoing isolated elective mitral valve replacement, the Mosaic bioprosthesis may offer better freedom from reintervention compared to the Epic bioprosthesis.
Mosaic may be associated with fewer reinterventions after mitral replacement; hypothesis-generating and requires randomized confirmation.
Background: Comparative studies of outcomes between different biological mitral valve prostheses are scarce. This study compares the late clinical results of valve replacement with the Epic and Mosaic bioprostheses. Methods: Patients undergoing isolated elective mitral valve replacement (MVR) between 2005 and 2019 were eligible for inclusion. Primary outcomes were freedom from mitral valve reintervention and overall survival. Inverse probability of treatment weighting and competing risk analyses were performed. Results: = .067). Structural valve deterioration was an uncommon indication for reintervention in the first 10 years postoperative. Conclusions: Clinical results of MVR with the Epic or Mosaic prosthesis are satisfactory. Our results suggest that the Mosaic bioprosthesis might offer better freedom from reintervention.

Tomšič et al. (2023) conducted a cohort in isolated elective mitral valve replacement. Epic bioprosthesis vs. Mosaic bioprosthesis was evaluated on freedom from mitral valve reintervention and overall survival (p=0.067). Mitral valve replacement with the Epic or Mosaic prosthesis yielded satisfactory results, with the Mosaic valve potentially offering better freedom from reintervention (P=0.067).
