Fenoldopam reduces BP in pediatric kidney disease but causes excessive hypotension in ~87% of patients.
Why the study?
Hypertensive crises in children with kidney disease require effective and safe management strategies, and the efficacy and safety of fenoldopam in this setting needed evaluation.

Excessive MAP drops with fenoldopam in pediatric kidney disease suggest dose caution; leaves open optimal parameters for prospective trials.
Partigiani et al. (2024) conducted a cohort in Hypertensive crises in children with kidney disease (n=74). Fenoldopam was evaluated on Reduction of mean arterial pressure not exceeding 25% of calculated drop after 8 hours of infusion. Fenoldopam reduced blood pressure in all pediatric patients with kidney disease after 8 hours, but 87% experienced a mean arterial pressure drop exceeding the 25% target, which was associated with higher median doses (0.4 vs 0.2 mcg/kg/min, p=0.002).
