Adding a septal electrode cuts atrial defibrillation thresholds by ~32% versus standard configuration.
Why the study?
The standard atrial defibrillation lead configuration uses electrodes in the right atrial appendage and coronary sinus, but the effect of adding an atrial septal electrode on atrial defibrillation threshold was unknown.
Absolute Event Rate: 0.86% vs 1.27%
p-value: p=<0.05

May guide future lead configurations for atrial cardioversion; leaves open whether threshold reduction improves outcomes or safety.
Zheng et al. (2000) studied Sustained atrial fibrillation (n=8). Additional electrode at the atrial septum (SP) vs. Standard RAA-->CS configuration was evaluated on Atrial defibrillation threshold (ADFT) delivered energy (p=<0.05). An additional electrode at the atrial septum significantly reduced the atrial defibrillation threshold compared to the standard RAA-->CS configuration (0.86 J vs 1.27 J; P<0.05).
