Does CA 125 have comparable diagnostic accuracy to NT-proBNP in predicting fluid overload in heart failure patients?
CA 125 levels greater than 33.3 U/mL offer diagnostic accuracy comparable to NT-proBNP for identifying fluid overload in patients with heart failure.
CA125 may offer a comparable, accessible alternative to NT-proBNP for fluid overload assessment in HF; cross-sectional data leave open the need for prospective validation.
Introduction: Congestion significantly influences the advancement of heart failure, and effectively managing fluid overload and pulmonary congestion is crucial. NT-proBNP, a well-established biomarker for heart failure, reflects ventricular wall stress. Its elevation correlates with increased fluid volume. Carbohydrate antigen 125 (CA 125), originally associated with ovarian cancer, has now garnered attention as a potential marker for fluid overload and pulmonary congestion in heart failure patients. However, determining the optimal CA 125 cutoff value for diagnosing this condition remains an area of variability across different research studies. While both biomarkers show promise, further research is needed to determine their comparative utility in predicting fluid overload. Factors such as sensitivity, specificity, and optimal thresholds should be considered. Study Objectives: 1. Assessmenthe prevalence of fluid overload in heart failure patients. 2. Diagnostic Accuracy: Identify the optimal cutoff point, calculate the AUC ROC, sensitivity, specificity of CA 125 and NTproBNP in predicting fluid overload. 3. Compare value of CA125 with NT-proBNP in predicting fluid overload in heart failure patients. Materials and Methods: Our cross-sectional study enrolled 121 heart failure patients at the HCMC Hospital of Rehabilitation - Occupational Diseases during the period from September 1, 2023, to July 1, 202. Results: Among the patients, 87 cases (prevalence: 71.9%) exhibited fluid overload. The mean age was 69.14 ± 12.96 years, with men comprising 51.2% of the cohort. The median CA 125 value was 29.28 U/mL (interquartile range: 22.57–37.24). The optimal CA 125 cutoff for predicting fluid overload was >33.3 U/mL, yielding an AUC ROC of 0.721 (95% CI: 0.632–0.799, p < 0.0001), sensitivity of 55.17%, and specificity of 85.29%. The optimal NT-proBNP cutoff for predicting fluid overload was >1929 ng/ml, yielding an AUC ROC of 0.725 (95% CI 0.632–0.799, p < 0.0001), with a sensitivity of 57.5% and specificity of 79.4%. Conclusion: CA 125 demonstrates reasonably good accuracy in predicting fluid overload in heart failure. CA 125 level >33.3 U/mL and NT-proBNP level >1929 ng/ml are predictors of fluid overload. Compare the value of CA 125 with NT-proBNP in predicting fluid overload by comparing 2 ROC curves, using the Delong method, the results showed no difference, p=0.941

Phan Thai Hao (2024) conducted a cross-sectional in Heart failure (n=121). CA125 vs. NT-proBNP was evaluated on Diagnostic accuracy (AUC ROC) for predicting fluid overload (p=0.941). CA125 and NT-proBNP demonstrated similar diagnostic accuracy in predicting fluid overload in heart failure patients, with an AUC of 0.721 and 0.725 respectively (p=0.941 for comparison).
