Key result
CRT-D is linked to ~66% lower mortality vs CRT-P in low-risk patients.
Why the study?
The applicability of the Goldenberg risk score to predict long-term mortality and identify CRT patients who benefit from defibrillator implantation was uncertain.
Cohort (n=638)
Hazard Ratio: 0.339
p-value: p=0.001
Low-risk CRT patients may derive short-term defibrillator benefit; hypothesis-generating for Goldenberg score-guided selection, prospective trials needed.

Barra et al. (2015) conducted a cohort in Ischaemic or non-ischaemic dilated cardiomyopathy (n=638). CRT-defibrillator (CRT-D) vs. CRT-pacemaker (CRT-P) was evaluated on All-cause mortality in patients with Goldenberg risk score 0-2 (HR 0.339, p=0.001). CRT-D was associated with lower all-cause mortality compared with CRT-P in patients with a Goldenberg risk score of 0-2 (HR 0.339, P=0.001), but not in those with score ≥3.
