Key result
Frailty affects ~45% of heart failure patients, supporting integrated GDMT and multidomain geriatric interventions.
Why the study?
Heart failure and frailty are increasingly prevalent with population aging, frequently coexist, and share pathophysiological mechanisms, motivating a synthesis of recent evidence on shared biological pathways and management strategies.
Do individualized multidomain interventions integrated with guideline-directed heart failure therapy improve patient-centered outcomes in older adults with heart failure and frailty?
Do individualized multidomain interventions integrated with guideline-directed heart failure therapy improve patient-centered outcomes in older adults with heart failure and frailty?
This review highlights that heart failure and frailty share biological aging pathways—including mitochondrial dysfunction, cellular senescence, inflammation, and altered myokine signaling—and recommends routine frailty screening and individualized multidomain management alongside guideline-directed heart failure therapy.
Supports frailty screening in heart failure; leaves open efficacy of integrated multidomain interventions pending randomized trials.
Purpose of Review Heart failure (HF) and frailty are increasingly prevalent with population aging, frequently coexist, and share pathophysiological mechanisms. This review summarizes recent evidence on shared biological pathways and management strategies in patients with HF and frailty. Recent Findings Emerging data suggest that accumulation of damaged cellular components, mitochondrial dysfunction, cellular senescence, and chronic inflammation contribute to both conditions. Altered myokine-mediated signaling further highlights skeletal muscle as an important peripheral link between HF and frailty and as a potential therapeutic target. Frailty is increasingly recognized as a clinically relevant measure and potentially modifiable treatment target. Current evidence, largely derived from small or single-center studies, suggests that individualized multidomain interventions, including exercise training, nutritional optimization, and geriatric co-management, may improve patient-centered outcomes. Summary Management should integrate guideline-directed HF therapy with targeted treatment of geriatric syndromes, guided by multidimensional assessment and individual patient goals. Multicenter trials in frail HF populations are needed to inform evidence-based management recommendations.

Miroslava Valentová (2026) conducted a review in Heart failure and frailty in older adults. Multidomain interventions and guideline-directed heart failure therapy was evaluated. Frailty affects approximately 45% of patients with heart failure and shares aging-related biological mechanisms, warranting integrated guideline-directed medical therapy and multidomain interventions.
