The Integrated Primary Care (ILP) screening process at UPTD Puskesmas Bojongasih was still conducted manually, resulting in inefficient services, longer processing times, and potential difficulties in managing screening data. This study aimed to design and develop an Integrated Primary Care Service Screening Application to improve service effectiveness and facilitate healthcare staff in managing patient screening activities. Data analysis employed a qualitative approach through direct observation, interviews with relevant staff, and literature review to identify system requirements and existing operational problems. System development followed the Waterfall method, consisting of systematic stages from requirements analysis and system design to implementation and testing. The application provides several essential features, including patient data management, screening procedures, screening results and history, data searching, and reporting. Testing was conducted using Black Box Testing involving 15 test scenarios, and all functions operated as expected and were declared valid. The developed application can help healthcare staff manage screening data more systematically, efficiently, quickly, and accurately while supporting improved service effectiveness at UPTD Puskesmas Bojongasih. Regular staff training and assistance, alongside further development of automatic notifications, data searching, and reporting features, are recommended to optimize application utilization and ensure sustainable implementation in daily healthcare services.
