140 patients with chronic heart failure (NYHA class II-IV), mean age ~53.5 years, 60% male, based in China. Excluded patients with severe liver and kidney dysfunction, acute myocardial infarction, or unstable angina.

Intervention
Linggui zhugan decoction (200 ml/day orally, divided morning and evening) added to Western medicine (furosemide, metoprolol, spironolactone, isosorbide mononitrate, trimetazidine, irbesartan) for 2 weeks.

Comparator
Western medicine alone (furosemide 20 mg/kg bid, metoprolol 25 mg/kg bid, spironolactone 20 mg/kg qd, isosorbide mononitrate 20 mg/kg bid, trimetazidine 20 mg/kg tid, irbesartan 150 mg/kg tid) for 2 weeks.

Clinical effectiveness, inflammatory cytokines (IL-6, IL-18, TNF-α), T-lymphocyte subsets (CD3+, CD4+, CD8+), and cardiac function (LVED, LVEF) at 2 weeks.surrogate

Co-administration of Linggui zhugan decoction and standard Western medicine for 2 weeks significantly improved clinical symptoms, cardiac function, and inflammatory profiles in patients with chronic heart failure.
