Implication

Supports cilazapril BP reduction without tachycardia and higher metabolite levels in renal impairment; leaves open dose optimization pending randomized trials.

Structured PICO

Does cilazapril reduce blood pressure and how do its pharmacokinetics differ in hypertensive patients with normal versus impaired renal function?

P
Population
12 hypertensive patients, including 5 with normal renal function (NRF) and 7 with impaired renal function (IRF)
I
Intervention
Cilazapril 1.25 mg orally once daily for 5 or 8 consecutive days
O
Outcome
Blood pressure, heart rate, serum ACE activity, and plasma level profiles of cilazapril and its active diacid up to 24 h following the initial and last dosesurrogate

Cilazapril is an effective antihypertensive agent, but its dose should be reduced in patients with severe renal impairment due to delayed recovery of ACE activity and higher plasma concentrations of its active diacid.
