PDE5 inhibitor T-1032 improves GFR and reduces renal ET-1 in dogs with pacing-induced heart failure.
Why the study?
In severe CHF, ANP does not sufficiently oppose exaggerated ET-1 actions due to accelerated degradation of cGMP by PDE5, contributing to renal dysfunction.
Does chronic PDE5 inhibition improve renal function and suppress renal endothelin-1 production in a canine model of congestive heart failure?

Hypothesis-generating in canine heart failure; leaves open translation to human renal outcomes.
Yamamoto et al. (2002) studied congestive heart failure (n=15). T-1032 vs. placebo was evaluated on renal function and renal production of ET-1. In dogs with pacing-induced heart failure, the PDE5 inhibitor T-1032 improved GFR and reduced renal ET-1 expression, with ET-1-positive cells negatively correlating with GFR (r=-0.802, P<0.001).
