139 consecutive patients successfully implanted with a CRT device for refractory congestive heart failure, mean age 68+/-9 years, 113 men (81.3%), 69% NYHA class III, 31% NYHA class IV, mean LVEF 21+/-6%, mean QRS duration 188+/-28 ms.

Intervention
Cardiac resynchronization therapy (CRT) with biventricular stimulation

Outcome
Positive response to CRT at 6 months (defined as alive, not re-hospitalized for management of CHF, and NYHA class decreased by 1 point, and/or peak VO2 or 6 min hall-walk increased by >10%)composite

The degree of QRS shortening during biventricular pacing is a strong independent predictor of clinical response to cardiac resynchronization therapy at 6 months.
