Implication

May support acute BP reduction with nifedipine; single case leaves open net clinical benefit and renin effects.

Key Points

  • Evaluate the hypotensive effect and plasma renin activity changes induced by sublingual nifedipine administration in hypertensive patients.
  • Administered 30 mg of sublingual nifedipine to 19 hypertensive patients, including 14 with essential hypertension as well as patients with malignant, renovascular, and primary aldosteronism forms.
  • Assessed changes in systolic and diastolic blood pressure alongside plasma renin activity following administration.
  • In essential hypertension (n=14), blood pressure decreased from 177 ± 24 to 123 ± 13 mmHg systolic and from 108 ± 12 to 80 ± 11 mmHg diastolic (p < 0.01).
  • Plasma renin activity in essential hypertension increased from 0.73 ± 0.62 to 1.50 ± 1.02 ng/ml/h (p < 0.05), with similar increases observed in malignant and renovascular hypertension.
  • In primary aldosteronism (n=2), blood pressure decreased, but plasma renin activity showed no increase.

Structured PICO

Does sublingual nifedipine reduce blood pressure and alter plasma renin activity in patients with hypertension?

P
Population
19 patients with established hypertension (blood pressure >150 mmHg systolic or >90 mmHg diastolic in the supine position after 30 min bed rest), including 14 with essential hypertension (mean age 47), 2 with malignant hypertension, 1 with renovascular hypertension, and 2 with primary aldosteronism. All patients were medication-free for at least 4 weeks before the study.
I
Intervention
Nifedipine 30 mg administered sublingually as a single dose.
O
Outcome
Changes in blood pressure (systolic and diastolic), heart rate, and plasma renin activity (PRA) measured up to 4 hours post-administration.surrogate

Sublingual nifedipine effectively lowers blood pressure in various forms of hypertension while reflexively increasing heart rate and plasma renin activity.
