Key result
BDF 9148 enhances ouabain inotropy by ~2.9 mN but increases toxicity.
Why the study?
The interaction between the Na+-channel activator BDF 9148 and the cardiac glycoside ouabain in failing human myocardium and its effects on inotropy and toxicity were not well understood.
Population
19 failing (NYHA IV) and 5 nonfailing human myocardium samples
Comparison
Ouabain alone vs ouabain after prestimulation with BDF 9148
Design
Ex vivo study of electrically driven human papillary muscle strips

BDF may accelerate ouabain inotropy and toxicity in vitro; leaves open whether combined Na+-channel activator-glycoside use warrants clinical caution.

Schwinger et al. (1992) studied Heart failure (n=24). BDF 9148 and ouabain vs. Ouabain alone was evaluated on Isometric force of contraction (FOC) (p=< 0.01). Prestimulation with the Na+-channel activator BDF 9148 significantly enhanced the inotropic effect of ouabain on isometric force of contraction (+2.9±0.4 mN; p<0.01) but also increased toxicity.
